2012 Summer Reading Program Statistics and Evaluation

 

Please fill in all blanks. This form MUST be submitted by Friday, September 14, 2012.

Children's Participation
Total number of children registered (how many reading records given out)
Total number of certificates awarded (number of children completing the program or their goal)
Estimated number of books read, or read to by all participants (total of books listed on their reading records)

OR

Estimated number of minutes spent reading by all participants (total of minutes listed on their reading records)

OR

Estimated number of pages read by all participants (total number of pages listed on their reading records)


Young Adult Participation
Total number of young adults registered (ages 12-18)
Estimated number of books read

OR

Estimated number of minutes spent reading

OR

Estimated number of pages read


Programs:
Total number of ALL children's programs held (storytimes, performances, book discussions, etc.)
Total number of attendees at children's programs (children and adults attending events with children)
Total number of YA programs held
Total number of attendees at young adult programs
