Summer Survey 2019

This survey is mandatory for all Vermont public libraries that received CSLP summer reading materials from VTLIB. We would also like to hear from libraries that offered a Summer Reading Program (SRP) but did not use CSLP materials. There will be a place to indicate this on the survey.

Your responses will help the Department of Libraries, partnering state agencies, and the Collaborative Summer Library Program to provide helpful resources and support for future summer reading programs. Your responses will also help demonstrate the importance of libraries as leaders in summer enrichment, literacy support, and workforce development.

Instructions: only one person from your library system should complete the survey. The survey must be completed in one sitting. If you have all relevant statistics and anecdotes before the start of the survey, completion should take approximately 25 minutes. The deadline for evaluation submissions is Monday, September 30, 2019.

Please double-check to make sure this is accurate. This is the email where your submission confirmation will be sent after you complete the survey.
